Each option includes prescription coverage and is effective first day of employment. Employees who are not fully vaccinated for Covid-19 and who are enrolled in the ASI health plan will be subject to a $100 per pay surcharge. Fully Vaccinated employees are not subjected to the surcharge. Dental Coverage through Delta Dental: Choose between two dental options. Each option includes diagnostic procedures, including exams, X-rays and teeth cleaning. Coverage is effective first day of employment. Vision Coverage through EyeMed: Vision coverage includes exams, frames, standard corrective lenses and contacts. Coverage is effective on first day of employment.
